CRM for field service with customizable online forms

Looking for a CRM that is useful for field service management. Specifically, I want my website "request a quote" form to automatically sync with the CRM. More specifically, this form needs to be quite customizable and allow prospects to submit pictures.

A little more information for those that are interested. I am in the the tree service industry and would like prospects to send us picture(s) of the tree(s) in question. This doesn't work with the go to CRM for our industry, Jobber.
