Personal CRM of sorts

I don't really need a "CRM" tool that does a lot of extra features. I'd like one that has both an android app and a web version. Other than that, I need something that will keep track of my contacts (I'm a Pastor doing visitation), and then in each one I'd like to be able to add new notes for each visit.

I've tried Notion, but there are two drawbacks 1. It doesn't work offline, and 2. If you have an address in there, you have to copy and paste it into Google Maps. I'd like one that will allow me to click the address and it automatically opens in Google maps.

Thoughts?
