Medical Startup looking for CRM/practice software

This feels like a long shot, but I run a virtual medical practice and have been building all our software in house. It hit me today that someone may have already built more robust software/CRM. If so any recommendations?

Here's what we do:

We have:

• a pool of patients
• a handful of our doctors that work with us
• a patient contacts us and we schedule an appointment with one of the doctors of their choosing
• we survey the patient after the phone call appointment
• we capture learnings and do lots of graphs on how many appointments we have, which doctors are selected, what the issues are and how happy every one is in the survey.

Basically any local general medical practice.

Is there anything that is customizable out there that a small practice can use?

Thanks! I would be mindblowingly grateful if anyone has any ideas – I don't have enough knowledge to find out other than googling sponsored posts on CRM which is a mine of false information. Some of you may know – who aren't pushing one solution. thanks!!
