Multi-option form that saves data in categories dependant upon answers

Hello!

I need a CRM for a form that asks the user to click one option of three and then dependant upon that is taken to another question where they have another set of options that need to be categorised and saved in a database in tables of those categories. What is the best CRM for this please?
